Release of Liability to be completed and signed by parent/guardian if student is under 18.

I, on the date below, intending to be legally bound hereby, the undersigned agrees and does hereby release from liability and to indemnify and hold harmless Christ Episcopal Church of Madison, Inc. and any employees, agents or representatives representing or regarding The Christ Episcopal Church Art Scholarship Program (SUCCESS). This release is for any and all liability for personal injuries (including death), property losses or damage occasioned by or in connection with the person(s) or entity being released.  

The undersigned further agrees to abide by all the rules and regulations announced by the person(s) or entity being released and /or its affiliate groups and vendors. Any visual art remains the property of the student. Christ Episcopal Church may use it in bulletins, as display during the recital, on their website, social media, or other promotional opportunities Any compositions or choreography created by students remains the property of the student. Music, voice, and dance students agree to allow Christ Episcopal Church to use audio and video recordings or still pictures of performances in bulletins, as display during the recital, on their website, social media, or other promotional opportunities. Recognition of the artist will be displayed in any instance the art is used for promotion by Christ Episcopal Church.
