Instructor Agreement Form
Please carefully read the agreement below.
Christ Episcopal Church has a passion for the arts. Art, dance, and music can provide inspiration, healing, and a medium for self-expression which is vital for personal well-being. We would like to enable students in our community on this path of learning and self-expression. The Christ Episcopal Church Arts Scholarship program will fund lessons for students who could benefit from financial support. The program will also provide an opportunity to showcase the students’ art during services and during a recital at the end of each program session.
The following will be completed by a representative of Christ Episcopal Church of Madison, Inc. (CEC).
Representatives of Christ Episcopal Church (CEC) agree to:

In addition to the forms above, the CEC representative will provide the following to the instructor:​

Collect and deliver signed forms to the Parish Administrator at CEC who will file the paperwork.

Review and approve students for admission to the upcoming session of the program.

Organize service dates for students to perform offertory pieces.

Approve payment to students and instructors (if applicable) for one service performance.

Plan a recital and inform the instructor of the date and time 60 days in advance of the recital.

Host the recital at Christ Episcopal Church, 506 Mulberry Street, Madison Indiana, 47250.

Provide necessary sound equipment, chairs, music stands, and lighting for the service performances and recital.

Approve payment for the instructor for the required lessons. Payments will be issued to the instructor or studio in advance at the start of each month for the session in progress. The CEC Parish Administrator will print the checks. They can be mailed or hand delivered.

The following will be completed by the instructor of the student's chosen art form.

Hold regular lessons with the student as per the agreement with the student.

Help the student prepare for the recital and service performances. There will be three performances. The instructor and student can decide on the music with minimal guidance from the program representative. The student can pair up with the instructor or other students in the program for all three performances or play solo. The service offertory music should be 3-4 minutes and be appropriate to the season of the church: Advent, Easter, Lent, Christmas, etc.

One service performance provided as a gift to Christ Episcopal Church.

One service performance is a paid performance where the student (and instructor if needed) are compensated ($50) by Christ Episcopal Church as Supply Musicians.

One piece of music to be performed at the recital at the end of the session. It is acceptable to reuse one of the offertory pieces for the recital.

Contribute to the selection of a piece of art that will be used in the recital and service performances.

Voice and music lessons will help students prepare for a particular piece of music to be performed at the recital or services.

Dance lessons will also contribute to a dance performance at the recital or services.

Art will be showcased at the recital and services; it could be displayed, incorporated into a bulletin, or shared digitally. The student should be able to describe how their art piece was created and what they want the viewer to feel when they see it.

Complete the Instructor Survey at the end of the program session.

Obtain a completed Student Survey at the end of the program session.
This is an agreement between the instructor (named below) and the representatives of Christ Episcopal Church of Madison Indiana, Inc. for the purpose of conducting the Christ Episcopal Church Arts Scholarship Program (SUCCESS).
